D-SNP: 18 plans
For people with both Medicare and Health First Colorado (Colorado Medicaid).
Boulder County covers Longmont, Boulder and the Boulder County side of Erie. This guide shows every Medicare Advantage plan CMS lists for the county in 2027.
Examples of plans with drug coverage and a $0 combined monthly premium for Parts C and D, by 2026 star rating and out-of-pocket limit
CMS Medicare Advantage and Part D Landscape, contract year 2027. A plan's fit depends on your doctors and prescriptions; Lisa checks both before you choose.
Every figure comes from the CMS Landscape file for contract year 2027. Special Needs Plans are counted separately.
Two ways to count "$0 premium" plans: 30 have a $0 Part C premium, and 27 have a $0 combined monthly premium for Parts C and D (22 of those include drug coverage; 5 have no drug coverage, so no Part D premium). The average premium figures likewise show the Part C premium and the combined Part C + D premium separately. Star ratings shown are 2026 ratings: CMS published 2027 ratings on October 8, 2026, and the tables on this site still show each plan’s 2026 rating until its data snapshot is refreshed. Lisa checks each plan’s 2027 rating with you before you choose.
PPO plans are the largest group here (13 of 33), and 5 plans have no drug coverage.
| Plan type | Plans | $0 Part C premium | Average Part C premium | Average out-of-pocket limit | Without drug coverage |
|---|---|---|---|---|---|
| PPO | 13 | 12 | $0.69 | $7,050 | 3 |
| HMO-POS | 12 | 10 | $20.17 | $5,192 | 1 |
| HMO | 8 | 8 | $0 | $5,713 | 1 |
Grouped by plan type, because type decides how the network works: HMOs expect you to stay in network, PPOs let you go outside it at a higher cost.
13 plans, 12 with a $0 Part C premium, average Part C premium $0.69, average out-of-pocket limit $7,050, 3 without drug coverage.

In the CMS Hospital General Information file, five acute care hospitals carry a Boulder County record: Boulder Community Health at 4747 Arapahoe Avenue in Boulder, Good Samaritan Medical Center at 200 Exempla Circle in Lafayette, AdventHealth Avista at 100 Health Park Drive in Louisville, and two in Longmont, Longmont United Hospital at 1950 Mountain View Avenue and Longs Peak Hospital at 1750 East Ken Pratt Boulevard.
Medicare Advantage networks are set plan by plan, so a plan can include one of these hospitals and leave out the one a few miles away. Before you pick a 2027 plan, write down the hospital you would go to in an emergency and the one your specialists use, and check both against every plan you are considering.
Lisa does that check for you, free and with no obligation, by phone or video for Boulder County clients, and then helps you enroll.
Boulder County runs Mountain Aging Services for older adults, people with disabilities, caregivers and veterans in the mountain communities. Resource specialists meet clients, learn their needs and build an action plan with them. There is a Northern Mountain Resource Specialist (720-766-8844) and a Southern Mountain Resource Specialist (720-616-9109), and the general aging line is 303-441-1617.
The county is candid about the limits: it says it generally cannot start a service where no provider exists, and that services depend on funding, staff and volunteers. That same reality shapes insurance choices in the mountains:
CMS lists losing Medicaid eligibility as a reason for a Special Enrollment Period: you can join, switch or drop a Medicare drug plan or a Medicare Advantage plan with drug coverage, and the window lasts 3 full months from the date you lose eligibility or are notified, whichever is later. Do not wait for the window to close.
For Health First Colorado questions, renewal notices and address updates, Boulder County Human Services is at 303-441-1000, with offices at 515 Coffman Street in Longmont, 3400 North Broadway in Boulder, and 1755 South Public Road in Lafayette. The same office can answer questions about the Medicare Savings Programs, which help pay Medicare premiums and, at the lowest income tier, deductibles and coinsurance. You can also manage your case on Colorado PEAK, where the county suggests opening your Mailbox and choosing how you want renewal notices delivered, so a letter that affects your coverage does not get lost.
CMS also lists what Original Medicare does not cover, including non-medical long-term care, most dental care and dentures, vision exams for prescription glasses, and hearing aids. When your Medicaid coverage changes, those gaps can appear overnight. Lisa can line up a Medicare plan that starts on time and, where it makes sense, separate dental, vision or hearing coverage.

Colorado totals are the sum of all 64 Colorado counties in the same Census release; a statewide median is not computed here. Survey estimates for small towns carry wide margins of error.
Medicare-certified hospitals the CMS Hospital General Information file records in Boulder County, with the CMS overall star rating. A rating is not network status: check each hospital against a plan.
5 Medicare-certified hospitals are listed, 1 with a 5 of 5 CMS overall rating; whether each is in a plan’s network is a separate question.

Your plan mailed an Annual Notice of Change by September 30, 2026. It lists what changes on January 1, 2027: premium, deductible, out-of-pocket limit, copays, drug list and network. Compare it line by line, or bring it to Lisa.
Counting plans with drug coverage, Special Needs Plans included, the Boulder County list goes from 62 plans in 2026 to 55 in 2027, and the average in-network out-of-pocket limit moves from $6,448 to $7,154.
| Measure | 2026 | 2027 |
|---|---|---|
| Plans with drug coverage (SNPs included) | 62 | 55 |
| With a $0 combined monthly premium for Parts C and D | 37 | 39 |
| Average in-network out-of-pocket limit | $6,448 | $7,154 |

| Maximum Part D deductible allowed by CMS | $615 | $700 |
| Part D out-of-pocket cap | $2,100 | $2,400 |
These counts cover plans with drug coverage, Special Needs Plans included, so they differ from the headline count of 33, which leaves out Special Needs Plans and includes plans without drug coverage. Clear Spring Health has no Medicare Advantage plan with drug coverage in Boulder County for 2027.

New to the Boulder County list for 2027: UHC Dual Complete CO-S4 (HMO-POS D-SNP), Select Health Medicare Essential (HMO), HealthSpring Preferred (HMO), HealthSpring TotalCare Plus (HMO D-SNP), HealthSpring TotalCare (HMO D-SNP), Humana Total Complete (HMO), Aetna Medicare Chronic Care (HMO C-SNP), Aetna Medicare Full Dual Extra Care (HMO D-SNP), DEVOTED C-SNP GIVEBACK EXTRAS 016 CO (HMO C-SNP), DEVOTED GIVEBACK EXTRAS 019 CO (HMO). Some of these carry the same or a similar name as a 2026 plan that ends (see the table above); they are new plan IDs, so members of the old plan must enroll in them.
If your plan is not renewing: you receive a non-renewal letter in October. You can pick a new plan during the Annual Enrollment Period (October 15 to December 7, 2026), and a Special Enrollment Period runs through the end of February 2027. Lisa confirms your dates and compares the replacement plans with you. The official Medicare & You handbook, mailed each fall, explains each option.
CMS rates each contract from 1 to 5 stars. This shows the 2026 rating for each of the 33 Boulder County plans. CMS published 2027 ratings on October 8, 2026; this table still shows 2026 ratings until its data snapshot is refreshed, and Lisa checks each plan’s 2027 rating with you before you choose. New plans and some small contracts are not rated.
No plan on the Boulder County list has a 5-star 2026 rating.
Plan choice follows the conditions you actually live with, because they decide which specialists and drugs you need covered.

An SNP is a Medicare Advantage plan limited to people who meet a condition. They are left out of the counts above because not everyone can join.
For people with both Medicare and Health First Colorado (Colorado Medicaid).
For people with qualifying chronic conditions: Cardiovascular Disorders; Chronic Heart Failure; Diabetes Mellitus.
For people who live in, or need the level of care of, a long-term care facility.

Medigap has no network: any provider that accepts Medicare accepts it, anywhere in the country. Premiums are higher than most Advantage plans, and in exchange your costs are more predictable. The Colorado Division of Insurance publishes Medigap premium charts for every company selling in the state, so you can compare the same plan letter across insurers.
Timing matters. Your 6-month Medigap open enrollment window starts when you are 65 or older and enrolled in Part B. Inside it, an insurer cannot turn you down or charge more for your health. Outside it, in most cases it can. See Medigap rules in Colorado and Medicare Advantage vs Medigap.
You must have Medicare Part A and Part B to join a Medicare Advantage plan, and live in the plan's service area, here Boulder County.
Seven months around your 65th birthday: the 3 months before, your birthday month, and the 3 months after.
October 15 to December 7, 2026. Choose or change plans for 2027; coverage starts January 1.
January 1 to March 31. If you are in a Medicare Advantage plan, you can switch plans or return to Original Medicare once.
Moving, losing employer coverage, gaining or losing Health First Colorado, or a plan not renewing can open a window outside the normal calendar.

Counting Medicare Advantage plans with drug coverage, Special Needs Plans included, Boulder County goes from 62 plans in 2026 to 55 in 2027. (The headline count of 33 plans on this page leaves out Special Needs Plans and includes plans without drug coverage, which is why the numbers differ.) The CMS crosswalk shows 10 plans ending with no similar plan in the county, 4 plans ending while the same company offers a plan with the same or a similar name under a new plan ID (members must enroll in it themselves), 3 plans folding into another plan automatically, and 10 new plans. Clear Spring Health has no Medicare Advantage plan with drug coverage in Boulder County for 2027. Your plan’s Annual Notice of Change lists what changes for you on January 1.
